Tell us about your business, what license you hold, and what products you’re interested in. The form below takes about 90 seconds.

Email your license

After you submit, send a copy of your cosmetology, esthetician, or wholesale license (PDF or photo) to hairbycatb@gmail.com. We verify within 1–2 business days.

Order tax-exempt

Once verified, you’ll receive custom tax-exempt invoices through PayPal for any order, and your account is flagged for skip-tax checkout going forward.
